I also understand that if I fail to meet the terms of the housing agreement or do not pay rent, information from this application may be reported to credit agencies. Only individuals listed on this application are permitted to occupy the premises. If the applicant is a minor, a parent or legal guardian must sign their full legal name below.
Consent for Tenant Screening Report*

I authorize Academy of Makeup & Arts Inc. to conduct a tenant screening report. This may include credit history, criminal background check, rental references, and employment verification. I understand this information will only be used to evaluate my housing eligibility and may be used again for lease renewals or tenancy updates.
