  • For the participant's parent/guardian: The above-named participant is permitted to take the listed medications, as prescribed, if needed during camp activities. I attest the participant is able to take these medications responsibly, as prescribed, with no assistance needed from the program staff/volunteers.

  • CAMPS ATHENA & PROMETHEUS - PARTICIPANT AGREEMENT, RELEASE, AND ACKNOWLEDGMENT OF RISK

  • I,*   *   (herein referred to as "Participant") understand this is a legal document and, in consideration of my participation in the Salt Lake City Fire Department's Camp Athena or Camp Prometheus (herein referred to as "Program"), hereby agree to release and discharge the Salt Lake City Fire Department and Salt Lake City Corporation (collectively "Salt Lake City"), on behalf of myself, my heirs, assigns, personal representative, and estate as follows:

  • 1. I understand and acknowledge that my participation in the Program entails certain inherent known and unanticipated risks, which could result in damage to or loss of property, illness or disease, physical or mental injury, or death. My participation in this Program is purely voluntary, and I elect to participate in spite of the risks.

    2.I understand the afore mentioned risks can be from, but not limited to: exposure to heat, smoke, theatrical smoke, vehicle exhaust, burning propane, noise, and weather conditions; risks associated with physical exertion, handling equipment, working in confined areas; risk of trips, slips or falls from working on uneven surfaces, from heights, in low light areas or with limited visibility.

    3. Being aware that this Program entails inherent known and unknown risks of injury to myself or others persons as a result of my actions, I expressly agree, covenant, and promise to accept and assume all responsibility and risk of injury, illness, disease, or death to myself or others or damage to and destruction of property arising from my participation in this Program.

    4. I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Salt Lake City, its agents or employees, and all other persons or entities from any and all liability, claims, demands, actions or rights of actions, which are related to, arise out of, or are in any way connected with my participation in this Program. I fully recognize that if I or any person is hurt, or property is damaged, while I am engaged in this Program, I will have no right to make a claim or file a lawsuit against the Salt Lake City Corporation, or its officers, agents or employees, except where such injury, illness, disease, or death is caused solely by the negligent acts or omission of Salt Lake City. I understand and agree that this paragraph shall not be construed to waive, enhance, or abrogate any statutory rights or any provision of the Utah Governmental Immunity Act, Utah Code Ann. § 63G-7-101, et. seq. or successor provisions thereof.

    5. I certify that I am physically and mentally capable of participating in this Program.

    6. I certify that I have sufficient health and accident insurance (e.g. hospital/medical insurance, student health insurance, short-term accident insurance, etc to cover any bodily injury or property damage I may incur while participating in this Program.

    7. I agree and understand that I will comply with all reasonable directions and instructions by the instructors during the Program.

    8. I understand that the instructors reserve the right to limit my participation if I am judged by the instructor to be physically or mentally incapable of meeting the requirements of participating in the Program.

    9. I agree that I will not possess, use, or be under the influence of alcoholic beverages, non- prescription controlled substances, or illegal substances while in attendance during any portion of the Program, while on Salt Lake City property, or using Salt Lake City equipment. I understand that I will be dismissed from the Program and may be barred from participating in future Programs for violation of this Agreement.

    10. I understand that there is "zero tolerance" for harassment, inappropriate actions, or unprofessional conduct. Participants may be removed from the Program for violating this prohibition at the discretion of Salt Lake City in consultation with group leaders. I understand it is my responsibility to report any inappropriate behavior of which I am aware.

    11. I understand that participation in this Program will require physical exertion that includes but not limited to: walking, sitting, lifting, bending, squatting, crawling, and climbing. Therefore, all Participants must wear appropriate clothing that allow for these actions. Participants will wear: pants that allow for ease of movement, Program issued t-shirt, socks, and sturdy shoes such as gym shoes or boots. Participants wearing shorts, skirts, or dresses or open-toe/open- heel shoes will not be able to participate. Pursuant to the "zero tolerance" harassment policy, garments with offensive language or images will not be permitted to be worn during the Program.

    12. I understand that certain protective gear will be issued for use or wear during the Program. It is understood that this gear will be worn by the Participant as directed by the Program's officers and training division staff.

    13. I agree that I will immediately notify the group supervisor or an appropriate Salt Lake City employee of any illness or injury sustained while participating in the Program. I agree that Program staff is authorized to make medical related decisions that are professionally deemed to be in the best interest of my health and safety if I am unable to communicate with them at the time of illness or injury.

    14. I hereby give my permission for Salt Lake City to use any photographs or video taken of me during my participation in the Program for future promotional or public relation purposes.

    15. As parent or legal guardian, I hereby consent and authorize media, such as print or television news programs, to take photographs or motion pictures of my child; or to produce videotapes, audiotapes, or other types of media productions that capture my child's name, voice, and/or image (any of the foregoing types of media are called the "Materials" in this Release form

    16. I also agree that the media may identify my child by name. (If you do not wish to have your child identified by name, please contact the event organizer by email and request to not have your child identified by name)

    17. I agree that both myself and my child are participating on a voluntary basis and neither I nor my child will not receive any payment from Salt Lake City or any third-party media for signing this Release or as a result of any publication of the Materials.

    18. I represent that the Participant is under 18 years of age.

    19. I understand that it is my responsibility to ask for any needed clarification regarding the contents, statements, or policies outlined in this "Participant Agreement, Release, and Acknowledgement of Risk." My signature below indicates that I have read this entire document, understand it completely, understand that it affects my and my child's legal rights, and agree to be bound by its terms.
