Camp & Participant Info

Release of Liability

In consideration of being allowed to participate in Studio Orange Baton and Dance activities, I hereby release, waive, discharge, and hold harmless the Dance Studio from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, accident, or loss that may occur while participating in any Dance Studio activity, whether caused by the negligence of the Dance Studio or otherwise.

Assumption of Risk

I understand that participation in dance classes, rehearsals, performances, and related activities at the Studio Orange Baton and Dance involves physical activity that may result in injury. I knowingly and freely assume all risks, both known and unknown, associated with participation in such activities.

Photo, Advertisement, and Social Media Release Form

I, the undersigned participant (or parent/guardian if participant is under 18), grant permission to Studio Orange to use photographs, videos, or other media featuring myself (or my child) for promotional and marketing purposes, including but not limited to:

1. Studio Website and Social Media: This includes use on platforms such as Facebook, Instagram, TikTok, YouTube, and other online or digital media.
2. Print and Online Advertising: This includes brochures, flyers, posters, banners, and other promotional materials.
3. Event and Competition Coverage: This includes recordings and photographs taken during classes, performances, competitions, and special events for educational and promotional use.

I understand that:

• These materials may be used indefinitely unless I provide written notice revoking permission.
• No monetary compensation will be provided for the use of these images or recordings.
• Studio Orange will not sell or distribute these images to third parties for commercial purposes without additional consent.

I release and discharge Studio Orange, its representatives, employees, and affiliated entities from any claims or liabilities related to the use of these images or recordings.

I understand that if I do not wish for myself (or my child) to be included in such media, I must submit a written request to opt out.

Camp Cost $125

• Pay $50 Deposit now (to reserve your spot)
• Remaining balance of $75 due June 6th

Please send payments as "friends and family" to avoid transaction fees

If Venmo asks to verify the last 4 digits of the phone number, the phone number will be 7943

In the note section of the venmo, please but your childs name as well as Camp Checkboard (ex. Christie Puhalla - Camp Checkerboard)

Failure to pay remaining balance by June 6th will result in the removal of participant.
