Liability Waiver and Release of Claims
I acknowledge and agree that by participating in this beach cleanup event organized by One Piece Please (OPP), I do so voluntarily and at my own risk.
I understand that participation in this activity may involve physical activity and inherent risks, including but not limited to, cuts, sun exposure, insect bites, slips, trips, falls, dehydration, or encounters with sharp or hazardous objects.
I hereby waive, release, and discharge One Piece Please, its officers, directors, sponsors, volunteers, and any partnering agencies from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in this event, including those allegedly attributed to the negligent acts or omissions of the released parties.
I understand and agree that this waiver is binding upon my heirs, assigns, and legal representatives. I further grant permission to One Piece Please to use photos or videos taken during the event for promotional purposes, unless I explicitly request otherwise in writing.
By signing this form, I affirm that I am 18 years of age or older or that I am the parent or legal guardian of the participant under 18 years of age and give consent for their participation.
