Form - GCISD Uniform Usage Agreement

By signing this form, you are assuming responsibility for all items listed and agree to keep all items in the best possible condition and to return them at the request of the director. You must pay for all items damaged or lost.You are expected to wear the uniform as instructed as a representative of both the Colleyville Heritage Panther Band and Colleyville Heritage High School. Alterations to the uniform of any kind are not permitted. You are to wear your marching shoes and black socks (no logo) with the uniform at all times. In the public eye, students are to wear the COMPLETE uniform at all times. This is to prevent any items from being lost.*

- For installment payments, 1st check will be for $300.00
Installment Payments
- Credit Card - 1st installment paid by 5/15, subsequent payments will be invoiced to Parent 1 email.
- Check - 4 separate checks given to treaurer by 5/15, with each check dated for the corresponding Due Date; checks for July/Aug/Sept payments will not be deposited until the 1st of that month.

Payments by check/cash can be brought to Spring Training Camp (4:30-6:30) on May 13, 14, or 15 or to the Parent Meeting on May 15.
