Application Instructions
Please complete the full application. Plan for 15–20 minutes to fill out and submit the form. A non-refundable $25 application fee is required at the time of submission. If your learner is accepted into the program, the application fee will be applied toward your enrollment deposit. Only one application per family is needed.
