Who is allowed to pick up your child after dismissal each day? Please be sure to put each adults first and last name. For safety reasons your child will not be dismissed to anyone who is not listed.*

Please suggest 1-3 friends your child would prefer to be with. This does not guarantee we can put them together but we will do our best to at least put your child in a group with one of them. If your child does not have a request then just say “anyone”. Additionally be sure if you have more then one child you put their name then their friends next to their name. That way we can see which of your children have listed which friends. *
VBS Safety Release

As the parent and/or legal guardian of the minor/minors mentioned below, I hereby acknowledge that the said minor/minors are under my care, custody, and control. I give my express permission for the minor/minors to participate in church activities at Corner Post Cowboy Church on Sundays,Wednesdays and other church events like Camp,VBS,etc.

Furthermore, I, as the undersigned parent and/or guardian, release and hold harmless Corner Post Cowboy Church and its representatives from any and all actions, damages, and liabilities arising from the participation of the minor/minors in church activities. This release is intended to absolve Corner Post Cowboy Church and its representatives from any liability related to the minor/minors' participation in church activities.
PHOTO RELEASE

By submitting this form, I give Corner Post Cowboy Church permission to photograph said minor/minors and use his or her pictures in the church’s digital and print communications. Corner Post Cowboy Church will never publish a child’s name or any other personal information with any of its publications. Additionally when submitting this form I am in agreement with the waiver and the form as a whole.
