Medical Disclosure: I have provided accurate and truthful medical information, and I understand that failure to disclose certain medical conditions may result in complications or adverse effects.
Health Risks: I understand that getting a tattoo carries inherent risks, including infection, allergic reactions, scarring, and other complications.
Aftercare Responsibility: I agree to follow the aftercare instructions provided by the tattoo artist to ensure proper healing and to minimize the risk of complications.
Tattoo Quality and Permanence: I understand that tattoos are permanent, and I accept responsibility for the final appearance of the tattoo. I understand that over time, tattoos may fade, change, or require touch-ups.
Liability Waiver: I release the tattoo artist and the tattoo studio from any liability for injuries, infections, or complications resulting from the tattooing process, including any adverse effects due to pre-existing health conditions.
Photo Consent: I give permission for photographs of my tattoo to be taken for promotional or portfolio use by the tattoo studio (optional).
Alcohol and Drug Use: I confirm that I am not under the influence of alcohol or drugs during this tattoo session, and I understand that intoxication may result in refusal of service.
Tattoo Design Confirmation: I confirm that I have approved the tattoo design, placement, and size, and understand that changes after the process begins may be limited.
