    2. Please complete your LifeApp form, including the required signatures. All submitted LifeApps with incomplete details will not be approved.

    3. Be sure you upload the correct LifeApp according to your choice below.

    4. You will need your GLC Trainee Number to fill out this form so we can properly file your LifeApp. If you don't have or have forgotten your trainee number, kindly contact GLC via Messenger for assistance.
