🏀 Halo Hoops Basketball Camp Participant Agreement 🏀

Acknowledgment

I, the undersigned, acknowledge and understand that participation in the Halo Hoops Basketball Camp (herein referred to as the “Program”) is intended to promote fundamental basketball skills and foster an enriched understanding of the game within a Christian environment.

Code of Conduct

I pledge to conduct myself, and ensure that my child conducts themselves, in a manner consistent with the values and expectations of the Program. This includes exhibiting positive sportsmanship and respect toward opponents, teammates, coaches, officials, and spectators. I will not engage in, nor permit my child to engage in, any harmful or negative behavior that could undermine the integrity or Christian principles of the Program. I further agree to encourage my family and guests to abide by these standards at all Program activities.

Consequences for Misconduct

I understand that if the behavior of a player, parent, or guest is deemed inconsistent with this Participant Agreement or the values of the Christian faith, disciplinary action may be taken. This may include immediate removal from a game, practice, or facility. Severe or repeated misconduct may result in permanent removal from the Program without refund.

Health and Physical Capability Attestation

I attest that my child is in good health and physically capable of participating in rigorous athletic activities, including basketball. I confirm that my child has had a sports physical examination within the past 12 months. I have disclosed all known medical conditions, allergies, or physical limitations to the Program and provided any necessary medical information or instructions.

Liability Waiver

As the parent(s) or legal guardian(s) of the participant named within this registration, I voluntarily grant permission for my child to participate in the Program. Recognizing the inherent risks associated with athletic activities, I voluntarily assume all risks related to participation. I hereby release, waive, indemnify, and hold harmless Angels Basketball LLC, St. Peter Lutheran Church and School, and their respective officers, employees, agents, and volunteers from any and all liability, injury, damages, or claims arising out of or related to my child’s participation in the Program, including but not limited to physical injury, illness, or allergic reactions.

Photo and Media Release

I hereby grant Angels Basketball LLC the unrestricted right and permission to use my child’s name, likeness, image, and/or appearance as captured in photographs, video recordings, or other media, in any and all media formats, publications, advertising, and promotional materials, including web-based publications, without compensation. I waive any rights to inspect or approve the finished product or any material wherein my child’s likeness appears. I also waive any claims to royalties or other compensation arising from the use of such media and release Angels Basketball LLC from all related claims, demands, and causes of action.

Communication Consent

I hereby authorize Angels Basketball LLC, including its representatives, coaches, and administrative staff, to contact me via phone call, email, and text message regarding matters related to the Halo Hoops Basketball Camp, the Angels Basketball program, and any future camps, clinics, or related events. I understand that standard messaging and data rates may apply for text communications. I acknowledge that I may opt out of non-essential communications at any time by notifying Angels Basketball LLC in writing.
