2025-2026 Faith Formation Enrollment Form

Welcome! We’re excited to have you join us for Faith Formation this year. Please take a moment to review the instructions below before filling out the form. The form follows the instructions.

Programs You Can Enroll In:
A Few Things to Know:
- One Form for All Programs: This form is for all our Faith Formation programs and
  sacramental preparation, including Youth Ministry.
- Personalized URL: If you’ve received a personalized URL, please check that your
  information is correct. If you’re new, fill out all the required fields.
- Save and Finish Later: If you need more time, you can save your progress and come back to complete it. Just make sure to click "Submit" when you’re ready.
- Early Bird Rate (RE Programs):
  - $200 per child if you enroll by June 30, 2025.
  - $300 per child if you enroll by August 1, 2025.
  - After August 1, children will be placed as space allows.
  - New families: pay $200 per child, no matter when you enroll.
  - A multi-child discount is automatically applied.
- Financial Assistance: If you need help with the fees, scholarships are available. Please reach out to Anne Marie Kaufmann at 202-903-2832 or akaufmann@trinity.org.
First Reconciliation/First Eucharist Preparation (for 2nd grade and older):
- Please review the sacramental readiness requirements on our Sacramental Preparation page.
- Children should have been enrolled in a faith formation program for at least one year
  before and during the sacramental year.
Confirmation Preparation (for 8th grade and older):
- Please review the Confirmation guidelines on our Confirmation Preparation page before enrolling.
- Children should have been enrolled in a faith formation program for at least one year
  before and during the sacramental year.
