At Advanced Practice Wellness Clinic, we are committed to providing high-quality care to all of our patients. To maintain an efficient schedule and reduce wait times for everyone, we ask that patients notify the clinic in advance if they are unable to attend their scheduled appointment. A patient must maintain an active card on file to adhere to the no-show policy.
1. DEFINITION OF A NO-SHOW
A "No-Show" is defined as a patient who misses a scheduled appointment without notifying the clinic at least 48 hours in advance.
2. CANCELLATION NOTICE:
If you are unable to attend your scheduled appointment, we ask that you notify the clinic at least 48 hours in advance. This allows us to offer the time slot to another patient in need of care.
3. CONSEQUENCES OF NO-SHOWS:
- First No-Show: You will receive a reminder notice of our No-Show policy.
- Second No-Show: A fee of $75 will be charged to your account.
- Third No-Show: A fee of $75 will be charged to your account. Patients will not be able to make further appointments; they will have to become walk-in patients ONLY and may have to wait for the next available time that day if there is one.
4. LATE CANCELLATIONS
If you cancel an appointment within less than 48 hours of your scheduled time, a late cancellation fee of $75 may apply, depending on the nature of the appointment.
5. SPECIAL CIRCUMSTANCES
We understand that emergencies and unforeseen situations can arise. If you miss an appointment due to an emergency or other extenuating circumstances, don't hesitate to contact our office as soon as possible to discuss your situation.
6. EXCEPTIONS AND APPEALS
Suppose you believe the No-Show charge is unjustified. In that case, you may submit a request for review to our office, and we will consider any special circumstances or medical conditions that may have contributed to the missed appointment.
7. REPEATED NO-SHOWS:
Repeated no-shows may lead to discontinuing care or a referral to another provider. We appreciate your cooperation in maintaining an organized and efficient practice. Our goal is to provide the best care for all our patients, and your adherence to our No-Show Policy helps us accomplish that.
