I certify that the information provided on this form is correct, and that I hereby authorize Carolina Smart Home (BFD Carolina LLC) to initiate automatic Zelle deposits to my account at the financial institution named above. It is my responsibility to notify Carolina Smart Home immediately (1-800-750-3718, Ext. 701) if I believe there is a discrepancy between the amount deposited and that reflected on my invoice.

Furthermore, I agree not to hold Carolina Smart Home responsible for any delay or loss of funds due to incorrect or incomplete information supplied by me or by my financial institution or due to an error on the part of my financial institution in depositing funds to my account. This agreement will remain in effect until Carolina Smart Home receives a written notice of cancellation from me or my financial institution, or until I submit a new direct deposit form to the Payroll Department.

If I decide to change my Zelle direct deposit information, I understand and agree that the terms of this authorization form remain in effect.
