- I am the owner of the animal(s) listed in this form, responsible for them, and have the authority to execute this consent. I authorize the veterinarian to complete spay/neuter surgery and/or their assistants to vaccinate my animal(s).
- My animal(s) do not have a chronic medical condition.
- My animal(s) is/are currently healthy and have not shown signs of illness (coughing, sneezing, vomiting, diarrhea, lethargy, loss of appetite, etc.) within the last 14 days.
- My animal(s) has/have not bitten a person within the last 10 days.

- I understand that Animal Care Services uses qualified staffing and approved materials for all procedures performed. Additionally, I understand that the risk of injury or death, although extremely low, is always present just as it is for humans who undergo surgery.
- If in the course of treatment, a condition is discovered which requires medical attention or an additional procedure, such as hernia repair or the administration of IV fluids, the attending veterinarian may, with sole discretion, perform such procedure. I consent to these procedures.
- I understand that as long as, in the opinion of the attending veterinarian, the animal is an acceptable surgical candidate, sterilization procedures will be performed regardless of the animal’s sex or medical condition (including pregnancy).
- I understand that the attending veterinarian, with sole discretion, can refuse to perform any procedure on any animal for any reason.
