Volunteer Release and Waiver of Liability

Please read this form carefully and sign it before participating in any volunteer activities with MNM Foundation, Inc.

I. Volunteer Information

II. Description of Volunteer Activities

I, the undersigned ("Volunteer"), desire to participate in volunteer activities with MNM Foundation, Inc. ("MNM" I understand that these activities may include, but are not limited to:

III. Waiver and Release of Liability

I understand that there are inherent risks and hazards associated with participating in volunteer activities, which could result in personal injury, illness, or property damage. I freely and voluntarily assume all such risks and hazards.

In consideration for being permitted to participate in these volunteer activities,I

hereby release, waive, and forever discharge MNM Foundation, Inc., its directors, officers, employees, agents, and volunteers (collectively, "Released Parties"), from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of my property, while participating in such activities, or while traveling to or from such activities, even if such loss, damage, or injury occurs as a result of the negligence of the Released Parties.

IV. Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, losses, damages, liabilities, costs, and expenses, including reasonable attorneys' fees, arising out of or relating to my participation in the volunteer activities, including but not limited to any claims arising from my own negligence or the negligence of others. This means that I will be responsible for any costs incurred by MNM Foundation, Inc. as a result of any claims or lawsuits brought against them due to my actions or inactions during my volunteer work.

V. Assumption of Risk

I understand that the volunteer activities may involve physical exertion and that I should consult with a physician if I have any concerns about my ability to participate. I affirm that I am in good health and have no medical condition that would prevent me from safely participating in these activities. I am responsible for my own medical insurance coverage.

VI. Photographic Release

I grant to MNM Foundation, Inc., its successors and assigns, the right and permission to photograph or otherwise record my image and voice during the volunteer activities. I agree that these photographs or recordings may be used for any legitimate purpose, including but not limited to publicity, advertising, and fundraising. I waive any right to inspect or approve the finished product.

VII. Governing Law

This Release and Waiver of Liability shall be governed by and construed in accordance with the laws of the State of Florida.

VIII. Severability

If any provision of this Release and Waiver of Liability is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

IX. Acknowledgment

I have carefully read this Release and Waiver of Liability and fully understand its contents. I am signing this document voluntarily and of my own free will.
