Facial Membership Agreement
This Membership Agreement (the “Agreement”) is made between the undersigned member (“Member”) and Arcane Esthetics LLC (“Provider” By signing this Agreement, the Member agrees to be bound by the terms and conditions outlined below:
1. Membership Term
• Initial Term: Membership is for a minimum period of three (3) months.
• Renewal: After the initial term, the membership will automatically continue on a month-to-month basis.
2. Cancellation Policy
• Early Cancellation: If the membership is canceled before the completion of the initial 3-month period, the Member will be charged the remaining monthly fees.
• Notice: A written 30-day notice is required to cancel the membership. Failure to provide notice will result in a charge equivalent to a full month’s fee of $99.00.
3. Appointment Policy
• Monthly Appointments: The Member is responsible for booking at least one appointment each month. Failure to do so will result in a charge of the membership tier price, with no carryover or transferability of the missed appointment. It is not the responsibility of Arcane Esthetics to ensure the Member's monthly appointment has been scheduled.
• Cancellation: Appointments must be canceled at least 48 hours in advance. If cancellations occur 24 hours or less before the scheduled appointment time, you may be subject to a cancellation fee of $25. If last-minute cancellations become habitual, you may be removed from the membership program.
• No-Show: A no-show will result in a charge of $50. If no-shows become habitual, you may be removed from the membership program.
• Cancellation and no-show fees are non-refundable and are separate from your monthly membership fee. Failure to pay the cancellation fee will result in permanent removal from the membership program.
4. Tardiness
• If the Member is more than 15 minutes late, the appointment will be rescheduled, and the cancellation fee of $50 will be charged.
5. Membership Freeze
• Members are allowed to freeze their membership once for a period of one month. Membership will automatically restart thereafter.
6. Payment Terms
• Payment is scheduled for the same day that you signed up for the membership. If you need to change your subscription date, please do so via email to hello@arcanepdx.com 30 days prior to your scheduled due date.
• If payment is declined, the Member will be contacted. Failure to provide an updated payment method within 3 (three) days will result in immediate removal from the membership program.
• If the invoice for the declined fee is not paid, the Member will not be allowed to book appointments at Arcane Esthetics.
7. Refunds
• No refunds will be issued under any circumstances.
8. Liability Waiver
• The Member acknowledges that Arcane Esthetics does not diagnose medical conditions and releases the Provider from any injury resulting from undiagnosed conditions during treatment. The Member is responsible for informing the Provider of any changes in medical condition. Arcane Esthetics is not liable for allergic reactions or intolerance to treatments. The Member is responsible for informing the Provider of any previous adverse reactions and sensitivities.
9. Disclaimer
• Arcane Esthetics is not responsible for any injury or loss of property while on the premises or participating in services.
10. Co-Signer Requirement
• Members under 18 years old must have a parent or legal guardian co-sign and be present during all appointments.
