  • St. Eugene Faith Formation Registration Form 2026-2027 Program Year

    Please fill out the form to register for faith formation classes. After completing this form the director will review and if accepted will send out an invoice to the registering guardians email. Registrations are not completed and class seat is not secured until full payment is made. Teens entering confirmation program must first attend a confirmation meeting with parent and pass the Basic Catholic Faith Knowledge assessment at the time of the meeting. The Catholic Knowledge Assessment helps us understand where each student is in their faith formation so we can place them in the program that best supports their readiness for Confirmation. Any child that does not pass with 100% will have to complete a refresher course before they can enter the confirmation program.

Una vez completado el formulario, la directora lo revisará y, si se acepta la inscripción, enviará una factura al correo electrónico del padre o tutor que haya realizado la inscripción. La inscripción no se considerará completada y la lugar en la clase no estará asegurada hasta que se haya completado el pago. Los adolescentes que se incorporen al programa de confirmación deben asistir primero a una reunión de confirmación con sus padres y pasar la evaluación de conocimientos básicos de la fe católica en el momento de la reunión. La prueba de conocimientos católicos nos ayuda a comprender en qué punto se encuentra cada alumno en su formación en la fe, para poder asignarle el programa que mejor se adapte a su preparación para la Confirmación. Cualquier niño que no apruebe con un 100 % deberá completar un curso de repaso antes de poder acceder al programa de confirmación.
