I understand that it is the family's responsibility to apply insect repellent to my child before arriving at school, when it is appropriate to do so. I also understand that when necessary, due to environmental factors, MLMS may apply DEET-free insect repellent to my child before going to the woods or other outdoor activities.

If I/we do not wish MLMS to administer insect repellent at all, I/we will submit that request in writing.
