REQUEST FOR TRAFFIC SCHOOL

I,   Defendant Name,  the above-named defendant, am requesting to attend and complete traffic safety school to amend my charge to a non-moving violation. By submitting this request, I am not contesting the violation charged in my citation. Further, I affirm the following statements are true and correct.  

1. I have not attended and completed traffic school within the past 12 months.
2. This citation was not the result of an accident.
3. This violation did not occur in a school zone.
4. I do not have a commercial driver’s license (Class A or B).

I understand that if I fail to provide a Traffic Safety School Completion Certificate to the Court within 30 days, a conviction for the original charge cited will be submitted to the Department of Motor Vehicles. Extensions for completing traffic school will not be granted.
