This will serve as your order form and Payroll deduction form if your 90-day probationary period is not met, either due to voluntary or involuntary departure. In the event of a departure within the 90-day probationary period, you will have a 3-hour grace period to return the provided uniform in good, unaltered condition.

A uniform deduction will always appear under' year-to-date' deductions. Please do not think we are deducting from each check for the same uniform.

If the uniform is not returned within the 3-hour time frame after departure, a $60 fee (per full set) will be deducted from your final check.

Full-Time - 30+ hours, get 2 full sets

Part- Time - up to 30 hours, get 1 full set
