Urban Fitness will be offering a Girls Youth Strength Class starting June 16 until August 6, 2025. Class will be from 430-530 PM on Monday & Wednesday. This class is for youth, ages 10-14 (middle school), and is designed to improve strength , speed and overall athletic performance. This program will improve their athletic ability, self-confidence and help establish their mindset around physical training for the future.

Medical Release and Authorization

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is also granted to the Urban Fitness staff and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
Media Release

I authorize Urban Fitness to use the above listed athlete in photographs and videos that may be displayed in their gym, their website and/or social media pages.
Cost & Payment Agreement & Options

Cost will be $25/session, 5 sessions for $105, 10 sessions for $190 or all summer ( 14 sessions) for $250 plus a gym membership. All must be paid prior to class.

Pre-payment will be accepted via Venmo at Eddie-Davila-10 or in person with card ,cash or check to Urban Fitness or via square link above. Payment must be received to hold your daughters spot in class. If you chose the drop in option you must contact Savana Melland at smellandpt@gmail.com or 406-671-2781 prior to class to make sure there is room that day.

There are no refunds on class packs purchased! If your child suffers and injury or illness that prevents them from further participating then the remaining balance will remain on their account as an account credit.
