New Parishioner Registration Form
We are excited to welcome you to the Church of the Ascension family! Please help us serve you better by filling out all of the relevant items with information for yourself and all other relevant individuals you are registering as part of your household. If you were a member at some point in the past and are re-joining our parish, it is likely that you do not need to fill out this registration form. Please call the parish office and we can update your information and make you active again in our database. If you are a young adult now and were registered with your family as a child, you do not need to fill out a new registration form. We will update your information when you call the parish office. Be sure to fill in your registration form completely with your address, email and especially birthdates of your children. This will help us to notify you when it is time for you to register your children for sacraments.
