    Introduction
    You are invited to participate in a research study (H25277) conducted by the Institute for Health Logistics & Analytics at Georgia Southern University. The purpose of this study is to understand what you know (knowledge), how you feel (attitudes), and how you act (practices) related to the connections between human, animal, plant, and environmental health (One Health).

    As part of our commitment to supporting the health and well-being of County Wexford, we’re inviting residents to share their experiences and everyday practices that contribute to a healthier community. The following questions are designed to explore how local people connect with their environment, care for animals, and protect their own health — all of which are vital aspects of the One Health approach. Whether it’s how you manage your garden, care for pets or livestock, or take steps to keep your family healthy, your insights will help shape future programs and initiatives that reflect the needs and strengths of Wexford’s communities. 

    This survey will take approximately 10–15 minutes to complete. Your participation is voluntary, and you may choose to stop at any time without penalty.

    Procedures
    If you agree to participate, you will complete an anonymous online survey regarding your knowledge, attitudes, and practices related to One Health. No identifying personal information will be collected (including IP addresses). We also will not collect consent forms with names tied to responses. Since data is anonymous, your data cannot be withdrawn after submission because we won’t know which one is yours.  

    Risks and Discomforts
    This study presents minimal risks. However, some questions may involve topics such as disease transmission, environmental health, or antibiotic use, which may cause mild discomfort if you’ve had a bad past experience. You may skip any question you do not wish to answer. Since this survey is conducted online and in-person, please be aware that while we take precautions to secure your data, absolute confidentiality cannot be guaranteed.

    Benefits
    There are no direct benefits to you for participating. Participating in this co-created survey offers you, and the local Wexford community, a valuable opportunity to shape additional initiatives that directly impact your health and environment. By sharing your experiences, local residents like you can help identify key concerns and priorities related to the connections between human, animal, and environmental health in the places you live, work, and play. This input will inform tailored strategies that support community well-being, sustainable practices, and improved public health outcomes. 

    Confidentiality
    All responses are anonymous and will be stored securely. Data will be retained for three years and used solely for research purposes. Aggregated results may be shared in reports or presentations, but no individual responses will be identifiable.

    Right to Withdraw
    Participation is voluntary. You may stop at any time without consequence. If you do not wish to participate, simply close the survey window if taking the survey online or inform the survey administrator nearby if taking the survey in-person.

  • The first few questions focus on antibiotic use because understanding how residents use and manage antibiotics is key to preventing antibiotic resistance.
  • Our next question is about waste management because understanding disposal practices will help us better protect local water sources, wildlife, and community health by reducing pollution.
  • Our next questions are about interactions with wildlife because these encounters can impact both human and animal health.
  • Our final question in the practices section is about coastal erosion practices because Wexford’s coastline is vital to the community’s environment, economy, and way of life.

    Section 3 focuses on your attitudes because your beliefs and perspectives shape the choices you make about your health, your animals, and your environment.
