By signing this Agreement, the Client agrees to commit to completing a minimum of four (4) to six (6) or more body sculpting sessions as part of their package purchase at a discounted rate. Client agrees to schedule and attend all sessions consistently, with minimal rescheduling, to achieve optimal results. Missed appointments or frequent cancellations may result in the discount being voided. Clients appointments should be within one week of each other and no later than 2 weeks apart.
To maximize results, the Client commits to the following throughout the duration of their program: Exercise regularly (minimum of 3 times per week/30 minutes of movement Restrict alcohol intake to 0 if possible, must refrain from drinking any alcohol 72 hours in advance of treatment and after
Maintain a healthy diet that supports fat loss and muscle toning as your lifestyle allows. Follow pre/aftercare instructions provided by the technician during/after each session.
CANCELLATION AND RESCHEDULING POLICY
24-hour notice is required for any rescheduling or cancellation. No Show appointments are considered cancellations without notice that may result in loss of discount eligibility and loss of deposit.

I agree to provide 24 hours notice of a cancellation or change in appointment time, or I will forfeit a treatment off my package since treatments are by appointment only. There are no refunds if I decide to stop treatments. The Client commits to actively participating in the program, maintaining healthy habits, and following guidelines to achieve the best possible results
