SBS Bangkok Application Form

To process an application we will need information on the last four years of education (for Secondary and Upper Primary students).

To complete your child’s enrolment, please upload clear scanned copies or high-resolution photos of the required documents listed below.

For Thai Nationals สำหรับผู้มีสัญชาติไทย

• Thai National ID card or Birth Certificate of the student
  บัตรประจำตัวประชาชนไทยหรือสูติบัตรของนักเรียน
• House Registration 
  สำเนาทะเบียนบ้าน
• Thai ID cards of both parents (and guardian, if applicable) 
  บัตรประชาชนไทยของบิดามารดาทั้งสอง (และผู้ปกครอง หากมี)
• Recent Student Photo
  รูปถ่ายนักเรียนล่าสุด
• Report Cards
  ใบรายงานผลการเรียน
• Guardian Document + Power of Attorney (if student resides with a guardian)
  เอกสารผู้ปกครอง + หนังสือมอบอำนาจ (ถ้านักเรียนอาศัยอยู่กับผู้ปกครอง)
• Photos of both parents (and guardian if applicable)
  รูปถ่ายของบิดามารดาทั้งสอง (และผู้ปกครอง หากมี)

For Non-Thai Nationals

• Student’s Passport
• Parent passports (Mother, Father, Guardian)
• Thai visa (if already issued)
• Recent Student photo
• Report Cards
• Reference Letter

Notes

• All documents must be signed either digitally or in hard copy.
