Ashe County Youth Football League Sign-Up Packet

Player Registration Information

1. I understand that participation in youth football involves inherent risks, including the risk of serious injury (such as fractures, sprains, concussions), illness, or death. I voluntarily assume all risks associated with my child's participation and take full responsibility for any injuries or incidents that may occur.

2. Emergency Medical Authorization

I authorize ACYFL volunteers, coaches, and officials to administer basic first aid and/or obtain emergency medical treatment for my child if necessary. In the event I cannot be reached, I authorize the league to act in my place to secure appropriate medical care, including transportation to a hospital.

 3. Release of Liability

I, for myself and on behalf of my child, heirs, assigns, personal representatives, and next of kin, hereby release, hold harmless, and indemnify Ashe County Youth Football League, its directors, officers, volunteers, coaches, sponsors, officials, and agents from any and all liability for injury, illness, death or loss/damage to personal property arising from or connected with participation in any ACYFL activity. 

4. Insurance

I understand that while the Ashe County Youth Football League carries a supplemental insurance policy, it is my responsibility as the parent/guardian to provide primary insurance coverage for my child. I confirm that my child is covered under a valid health insurance policy.

5. Code of Conduct Acknowledgment

I acknowledge that I have read and agree to follow the ACYFL Parent Code of Conduct below, and | will ensure my child complies with player conduct expectation.

6. Media Usage

I understand and agree that Ashe County Youth Football League may use photographs, video, and/or audio recordings of my child taken during league activities for purposes including but not limited to promotional materials, social media, local media coverage, and the league's website. No compensation will be provided for the use of these materials.

7. Football Equipment Agreement

The following equipment will be provided by Ashe County Youth Football League:

 

- Practice Jersey (Value: $30)

- Helmet ($200)

- Shoulder Pads ($100)

- Game Jersey ($75)

- Game Pants ($25)

I agree to return all equipment in good condition at the end of the season. If any item is lost, damaged beyond regular use, or not returned, I understand I will be responsible for the replacement value listed above.

 

PARENT CODE OF CONDUCT

- I will model respect and sportsmanship for all players, coaches, officials, and spectators.

- I willnot approach coaches on game day or during practices. Any concerns must be addressed by setting an appointment 

- I will not engage in negative, confrontational, or disruptive behavior at games or practices.

- I will support my child, their teammates, and coaches with encouragement and positivity.

I understand that failure to comply with this Code of Conduct may result in removal of my child, myself, or both 

 

REFUND POLICY

I understand that the registration fee for participation in Ashe County Youth Football League is non-refundable, regardless of player withdrawal, dismissal from the program, or changes in availability.

 

Player Documents

I agree to provide a copy of my child's birth certificate and a current physical form before the first practice on

 

July 10th, 2026. Failure to submit these documents to the team mom by that date will result in my child being ineligible to participate. No refund will be issued. If the physical expires during the season, I understand that I am responsible for submitting a new one before my child may continue participating. 

(The league will be providing a date to receive physicals and players may participate if they plan to attend this in August as long as they have provided Birth Certificate and signed liability waiver.)

 

LEAGUE DISCRETIONARY POWER

I acknowledge that Ashe County Youth Football League staff reserve the right to withhold a player's participation in practice or games if they believe the player's health or safety may be at risk. An explanation will be provided to the parent/guardian if such a situation arises.

 

I understand that the registration is not valid until Ashe County Youth Football League has received 100 dollar payment unless other arrangements have been made and approved by Ashe Youth Football
