**“ROAR” Summer Band Camp 2025
 Combined Release of Liability & Assumption of Risk 

By signing below, I give permission for my child to participate in all activities associated with the “ROAR” Summer Band Camp administered by Beckford Band Camps and the Florida Memorial University (FMU) Department of University Bands (collectively, “the Camp”).
I understand that these activities—including travel to and from rehearsals, performances, and off-campus excursions—involve normal physical exertion and, as with any athletic or music program, carry some inherent risks of injury (from minor bumps or sprains to more serious incidents, though such events are uncommon).

In consideration of the Camp’s acceptance of my child’s registration, I voluntarily assume those risks and hereby release, waive, and hold harmless FMU, Beckford Band Camps, the Department of University Bands, and their officers, employees, agents, volunteers, sponsors, advertisers, and owners or lessors of any premises used, from any and all liability, claims, or demands arising out of or related to my child’s participation in—or travel connected with—any Camp session, except where caused by gross negligence or willful misconduct beyond their control.

Camper Expectations

As a registered participant in the “ROAR” Summer Band Camp (June 22 – 28, 2025), I understand that:

I am expected to attend all rehearsals, performances, and scheduled trips listed on the camp calendar.

Trips will be in-state, single-day excursions that do not require overnight lodging.

End-of-camp performances will also occur in-state and will not require overnight lodging.

I will comply with all rules and instructions issued by camp staff and counselors.

Failure to follow those rules or to remain under supervision voids any liability protection otherwise afforded to me.

Medical Release & Emergency Treatment Authorization

I, the undersigned parent or legal guardian of the above-named camper, hereby authorize qualified, licensed medical personnel to diagnose and treat my child in the event of an emergency when— in the attending professional’s judgment—prompt action is necessary to protect my child’s life, health, or comfort.

Specifically, I authorize:

Medical care: examinations, X-rays, laboratory tests, immunizations, and any minor surgical or medical procedures deemed immediately necessary.
Major care: if a serious illness, significant injury, or the need for major surgery arises, every reasonable effort will be made to contact me before treatment begins; however, if time is critical, I authorize physicians to proceed without prior consultation.
Pre-hospital aid: Directors, coaches, team parents, and other authorized staff of Beckford Band Camps / “ROAR” Summer Band Camp may administer first aid or arrange emergency transport until my child reaches a medical facility.
This authorization is valid for the full duration of my child’s participation in camp activities and may be revoked in writing at any time. I execute this release freely and solely to ensure that, in my absence, my child can receive timely medical attention necessary for their health and safety.

“ROAR” Summer Band Camp – Media Release

I hereby grant “ROAR” Summer Band Camp and its affiliates—including Florida Memorial University, the Department of University Bands, and any authorized media partners—the irrevocable right to photograph, film, record, and otherwise capture my image, likeness, voice, and/or performance. I understand that these materials may be used, reproduced, edited, and distributed—now and in the future—for promotional, recruiting, informational, or educational purposes in any medium (such as video, email, printed publications, newsletters, magazines, websites, and social media) without further notice or compensation.

I waive the right to inspect or approve any finished product or promotional copy that may include my image or likeness, and I release the Camp and its representatives from any claims arising from such use.

I further acknowledge that:

If I am 18 years of age or older: I have read and fully understand this release.
If I am under 18 years of age: My parent or legal guardian has read and fully understands this release and grants permission on my behalf.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
