I hereby acknowledge and agree to the terms and conditions set forth herein in relation to my participation in the volunteering activities offered by Suzanne CAPA (Chinese American Parents Association of Suzanne Middle School, hereafter referred to as "the Host"). I acknowledge that this program is independent and not affiliated with Suzanne Middle School or Walnut Unified School District.
1. Assumption of Risk: I recognize that participating in the volunteering activities may involve certain risks, including, but not limited to, the risk of physical injury, illness, or property damage. I willingly and voluntarily assume full responsibility for these risks.
2. Release and Waiver: In consideration of my participation in the activities, I hereby release and forever discharge Suzanne CAPA, Suzanne Middle School, Walnut Unified School District, and their respective officers, directors, volunteers, and agents from any and all claims, liabilities, actions, suits, demands, costs, and expenses (including attorney's fees) which arise from or are connected in any way to my participation in the program. This release and waiver extends to, but is not limited to, claims for personal injury, illness, or property damage.
3. Confidentiality: I agree to keep all related information confidential and not disclose any personally identifiable information about other participants.
4. Media Release: I grant the Hosts and their agents the right to use, reproduce, and/or distribute photographs, video, or audio recordings of me or my child participating in this learning activities for promotional purposes, without compensation or notification.
5. Agreement to Abide by Rules: I agree to abide by all rules and guidelines set forth by the Hosts in the learning activities. If a participant's behavior disrupts the hosts' instruction or the learning environment for other participants, after two warnings and notifications to parents without improvement, the participants may be dismissed from the program.
6. Governing Law: This waiver shall be governed by and construed in accordance with the laws of California. Any disputes arising from or in connection with this waiver shall be subject to the exclusive jurisdiction of the state and federal courts located in Los Angeles, California.
7. Acknowledgment:
I have read this waiver and fully understand its terms and conditions. I acknowledge that I am signing this waiver voluntarily and with full knowledge of its legal consequences.
