By signing below, I acknowledge and agree to the following on behalf of myself (or my child):
Assumption of Risk:
I understand that participation in any Homegrown Select Lacrosse activity – including but not limited to training sessions, clinics, tryouts, tournaments, competitions, and team events – involves physical activity and the use of lacrosse equipment, and may result in injury. I voluntarily assume all risks associated with participation.
Release of Liability:
I hereby release and hold harmless Homegrown Select Lacrosse Club, its coaches, staff, volunteers, affiliates, and event organizers from any and all liability, claims, demands, or causes of action that may arise from participation in any Homegrown Select Lacrosse activity, whether caused by negligence or otherwise.
Medical Consent:
In the event of illness or injury, I authorize Homegrown Select Lacrosse Club staff or volunteers to provide or seek necessary first aid or medical care for myself (or my child). I assume full financial responsibility for any medical services rendered.
Photo/Video Release:
I grant permission for Homegrown Select Lacrosse Club to take and use photographs and/or video recordings of the participant for promotional, educational, or organizational use without compensation.
Code of Conduct & Event Rules:
I agree to abide by all team rules, event guidelines, and the Homegrown Select Code of Conduct. I understand that failure to do so may result in removal from participation without refund.
