I give permission to the Concord VBS director, Christina Pressley and Preschool Directors, Lori Bigalow and Lori Schofield, to make whatever emergency measures (ie: first aid, disaster evacuation) are judged necessary for the care and protection of my child while participating in Concord VBS 2025 at Concord Church and Preschool.

In such case of an accident, every attempt will be made to contact parents and emergency contact listed.

In case of a medical emergency, I understand that my child will be transported to the nearest care facility by the local emergency unit for treatment if the local emergency resource (police, rescue squad) deems it necessary.

It is understood that in some medical situations the directors will need to contact the local emergency resource before contacting the parent, child’s physician, and/or other adults acting on the parent’s behalf.

Please submit registration and fee by July 1, 2025

REGISTRATION FEE: $30 per child ($75per family) Scholarships available

Online Payment: www.concordpc.org
- Go to the home page and click on “Give” then “Donate” in the drop down menu.
- Click on the yellow box that reads “Donate to Concord’s Ministries”
- Enter the dollar amount, choose “Youth Fund” in the drop down menu, choose payment method (PayPal, credit/debit)
- In “Write a Note” enter VBS 2025

I give permission for my child's photo to be taken during Concord VBS 2025 and used in church communications including the Concord Connects e-newsletter, www.concordpc.org website and Concord Facebook page. (If permission is NOT given and signature is not received, any group photos that include your camper will have their face blurred out.)
