Once you have finished and submitted your questionnaire, we will be replying back to your questionnaire (via email) and requesting several photos. Photos will include references for your tattoo idea, location of where you’d like your tattoo placed, and reference photos of tattoos that Danny has done. I acknowledge that if I do not follow the photo requirements in the reply email that my request will NOT be accepted.* This field is required.

*Please read this section very carefully before moving on** This field is required.In the next box, we will ask about your appointment availability. By selecting “I agree to the terms above”, you agree that your availability is accurate for the months that we are booking and should your availability change, you will notify us. When selecting your availability, you must choose at least TWO “All Day” availability options. We cannot guarantee Friday availability to anyone. We will be booking you based on the availability and the schedule you have sent us. If you cannot do the date we send you, we may not have an alternate date to offer you as we have other clients waiting for an appointment.
