Every day we we’ll go on a fun adventure to discover and collect the most valuable treasures that never lose it’s value within time. Parent/Guardian Consent and Release of Liability

I, parent /caregiver give permission for my child which name is on the form, to participate in all activities organized by Mill Creek Christian Assembly during the camp at Brightwood Ranch (Hope Mission).
I understand that while all reasonable safety precautions will be taken, there are inherent risks involved in travel and camp activities. I assume full responsibility for any risks and hazards incidental to my child’s participation.

I agree to release, indemnify, and hold harmless Mill Creek Christian Assembly, Hope Mission, and all camp leaders, volunteers, staff, and representatives from any and all claims or liability for injury, accident, or illness that may occur during travel to, participation in, or return from camp activities.

Medical Concent

In case of injury or medical emergency, I authorize the camp staff to provide first aid care to my child.

I authorize a licensed medical professional to provide diagnosis and treatment in the event of a medical emergency that requires immediate care to prevent serious harm or discomfort.

Social Media Concent

I understand that photos and videos will be taken during the camp by the MCCA Church Camp Team. If I do not wish for my child to appear in any photos or videos, I will contact the Camp Director, Sofia Bihun, directly prior to the start of the camp.

By signing below, I acknowledge that I have read, understand, and agree to the terms of this consent and release.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
