At the end of the assessment: 

1) Wait until a "BLACK SUMMARY PAGE" pops up at the end of the video. To know what the page looks like, refer to the picture (Reference 2) below this text. Click on the "Submit Answers" button to ensure that your answers are submitted for this assessment. 

2) After clicking on "Submit Answers". Take a screenshot of the "BLACK SUMMARY PAGE" and save it onto the device used to complete the "360^o Virtual Tour of Treament rooms at the Specialist Outpatient Clinics (SOC)." 

3) Upload the screenshot onto the JotForm named "VMHR: Clinic-Interactive Book Submission."
