Informed Consent and Acknowledgement

For Participants Aged 18 and Below (Parental/Guardian Consent)

I hereby give my approval for my child’s participation in any and all activities prepared by Tampines Floorball Club during the TFC-CLASSIC Summer Floorball Camp 2025. I assume all risks and hazards incidental to the conduct of the activities, and release, absolve, and hold harmless Tampines Floorball Club, SC Classic, and all their respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Tampines Floorball Club, SC Classic, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

For Participants Aged 18 and Above

I hereby confirm my voluntary participation in all activities organized by Tampines Floorball Club during the TFC-CLASSIC Summer Floorball Camp 2025. I acknowledge and assume all risks and hazards associated with travel to, participation in, and return from camp sessions. I release and hold harmless Tampines Floorball Club, SC Classic, and their respective officers, agents, coaches, and representatives from any liability for injuries or losses sustained during the course of the program.

I further waive all claims against Tampines Floorball Club, SC Classic, all coaches and affiliates, participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the event.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
