I-9 Eligibility Form

I9 form - MUST be completed by ALL NEW Coaches. I9 is a paper document that requires validation of documentation (preferably a current US passport) by Craig Forsyth, Kate Patil, or Carmen Fox. It is recommended that an appointment be made to do this with Craig in the AAFSC Office to deliver signed form and validate documents. Due within 3 days of first day of work or July 1, 2025 whichever is later.

If you are a current coach. Please upload your Green Light compliance screen shot(s) and insurance. YOU are responsible for contacting us for a renweal of documents if your address changes, ect.
