2025 SMLCA Baseball Camp Wavier
I hereby give my consent for my child(ren), named in the above registration form, to participate in the SMLCA Ospreys 2025 Baseball Camp. All personnel associated with the program shall not be held liable for any injury whatsover that my child(ren) may sustain in the activities thereof. I also certify that I know of no physical problems or conditions of my child(ren), which would impair participation in said program/camp. In the event my child is injured, I authorize the coach, director, or their representative to secure first aid and/or the services of any legally qualified physician or hospital, and i agree to assume all financial obligations connected to such incident.
