1. No-Call/No-Show Definition: A “No-Call/No-Show” occurs when a patient
fails to arrive for a scheduled appointment without prior notice.
2. Late Cancellation Definition: A “Late Cancellation” is when a patient
cancels their appointment with less than 24 hours’ notice.
3. Fee Structure: If you fail to show up for an appointment without prior notice or cancel/reschedule within 24 hours of your scheduled time, a prepayment of the exam fee ($63.54*) will be required for your next appointment.

4. Repeated No-Shows: Multiple no-shows may result in scheduling restrictions or discharge from the practice.
