Hold Harmless Agreement
Living Hope Bible Church
I/We agree to hold harmless, Living Hope Bible Church of Eldon, Iowa, and its members, officers, leaders and/or Pastors and teachers for any injury or death which may for whatever reason, occur during church related activities, including but not limited to: youth meetings, church outings, youth trips, picnics, recreation, fund raising activities, van transportation or any other function for which the Living Hope Bible Church has obtained parental consent, or any activity which a parent or guardian would normally allow their child to participate in, and which would not normally require a specific parental/guardian release.
I/We as the parent or legal guardian of the above named minor child(ren), do hereby give our permission for he/she to participate in the described activity associated with this specific release form. In granting this permission, I/we release the Living Hope Bible Church from liability. We also grant permission to the person in charge of this activity on behalf of the Living Hope Bible Church to take whatever reasonable course of action may be deemed necessary to insure the safety and disciplinary orderliness of this activity. I/We further authorize the Living Hope Bible Church to seek whatever medical or other treatment which may be deemed necessary during the time the below named minor child is involved in this activity.
Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Living Hope Bible Church and Forest Lake Camp have put in place preventative measures to reduce the spread of COVID-19; however, Living Hope Bible Church and Forest Lake Camp cannot guarantee that you will not become infected with COVID-19. Further, participation in any activity could increase your risk of contracting COVID-19.
By participating in programs, services, and activities held at Forest Lake Camp, you agree to the following:
On behalf of yourself and the participants of your group/organization, you hereby release, covenant not to sue, discharge, and hold harmless Forest Lake Camp and Conference Center, its employees, agents, and representatives, of and from all liabilities, claims, actions, damages, costs or expenses of any kind
arising out of or relating to your participation in our programs, services or activities. You understand and agree that this release includes any claims based on the actions, omissions, or negligence of this organization, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any such program, service
or activity.
For your own safety and the safety of others, please consult the latest CDC COVID-19 Guidelines prior to your arrival on our premises. At the discretion of the Camp Management staff, you and your group may be subjected to temperature checks while on the campgrounds and if it is determined that you have a temperature of 100.4 or higher, you will be askedto leave immediately. Temperatures will be taken with a contactless thermometer.
Restroom/Cabin Assignments
The restroom and cabin assignments will be given according to the child's God created biological sex. God-made boys will use the boy’s restroom and stay in the boy's cabin, and God-made girls will use the girl’s restroom and stay in the girl's cabin. By not following these guidelines, a child will not be able to attend.
This release is for:
2025 Vacation Bible School - June 22nd - 26th (6:00 - 8:00 PM)
2025 Victory Sports Camps - July 14th - 18th (10:00 AM - NOON)
2025 Bible Camp at Forest Lake Camp (August 4th - 6th) or (August 6th - 8th)
