To sign a waiver, you need to be at least 18 yrs old or older. If you are a minor (17 yrs old and younger), please have a guradian fill out this form. You can have your guradian or parent sing online nltag.com or jurassickarts.com

I understand that participation in activities including, but not limited to:
Bungee trampoline, drift kart/go-kart racing (indoor/outdoor), arcade games, virtual reality, simulators, ride-on vehicles, parties, contests, and events involves inherent risks, including:

Falls, collisions, and equipment malfunction
Serious injury, disability, or death
Emotional distress and property damage
These activities may involve running, jumping, climbing, flipping, and driving, increasing risk.
I voluntarily assume all risks, known and unknown.

Entiendo que participar en actividades que incluyen, entre otras:
trampolín bungee, go-karts/drift karts (interior/exterior), laser tag, juegos de arcade, realidad virtual, simuladores, vehículos recreativos, fiestas y eventos, implica riesgos inherentes, incluyendo:

Caídas, colisiones y fallas del equipo
Lesiones graves, discapacidad o muerte
Daños emocionales y a la propiedad
Estas actividades pueden incluir correr, saltar, escalar y conducir, lo que aumenta el riesgo.
