You’ll then be prompted to complete a separate form for each tick so our lab can accurately process your results. Please note: Each additional tick submitted will incur a lab testing fee of $94.99.

Please provide information about your tick sample(s) to be mailed for testing. Tick samples do not need to be alive or preserved in any specific way to be tested. If you’re submitting more than one tick, complete the questions for the first tick, then use the prompt at the bottom of the form that asks: “Do you have any additional tick(s) to add to your order?” You can add up to 5 additional ticks, and each additional tick submitted will incur a lab testing fee of $94.99.

When identifying the source of the tick, we refer to the person or animal it was found on as the “host.” If the tick wasn’t attached, please let us know where it was discovered—common locations include clothing, furniture, or outdoor areas.
