Safety is a top priority. This form helps clarify responsibilities so everyone can participate with confidence.
By checking the box below and signing this form, I acknowledge and agree to the following on behalf of my child:
I am voluntarily allowing my child to participate in Richmond Floorball Club (RFC) activities, including the Learn to Play program, and understand that participation involves physical activity and inherent risk, including but not limited to falls, collisions, or incidental contact with sticks or equipment.
I understand that, despite safety protocols, injuries or incidents may still occur due to the physical nature of the sport and interaction with others.
I certify that my child is in good physical condition and has no known medical reason that would prevent safe participation. I agree to inform RFC in writing of any allergies, chronic conditions, or limitations that may affect my child’s experience — and will also notify RFC staff on-site.
I understand that I assume responsibility for any injury, damage, or loss my child may incur while participating. I agree to waive any claim of liability against RFC, its volunteers, venues, sponsors, and partners, even in cases of ordinary negligence, to the fullest extent permitted by law.
In the event of a medical emergency, I authorize RFC personnel or volunteers to seek and obtain appropriate treatment for my child if I am not immediately reachable.
I understand that RFC may capture photos or video for promotional purposes. I consent to the use of my child’s image, likeness, or voice unless I notify RFC in writing before the start of the program.
This waiver applies to the full duration of the Learn to Play program unless revoked in writing.
I confirm that I am the parent or legal guardian of the participant and have full authority to agree to the terms above on their behalf.
