PARENT WAIVER INFORMATION

My child, listed below in the applicant section of this form, has my permission to audition to become a Dancer at Akademy Funktion Dance Collective. I understand that he/she must abide by the rules and regulations set forth by the Directors and the Koaches of Akademy Funktion Dance Collective. I understand that all forms attached and $25 Audition Fee must be completed by June 01,2025, or my child will not be allowed to audition. I understand that my daughter/son will be evaluated by qualified judges, and we agree to abide by the decision of the judges. I am interested in being a Kompetition Dancer at Akademy Funktion Dance Collective. I understand the risks stated above. If elected, I promise to abide by the rules and regulations set forth by the Directors & Koaches.
