The following letter will be shared with the service providers that you list below (which may include school personnel, therapists, and others) and gives your consent to information-sharing between Craftwell Education and your child and family's providers.
To Whom It May Concern,

I/we have engaged Craftwell Education as an educational consultant for my/our child, First Name* Last Name* . I/we authorize and encourage open communication between Craftwell Education and your institution or practice to support my/our child's academic development, social-emotional well-being, and overall success. This includes candid and unrestricted discussions regarding academic performance, personal growth, emotional health, and other factors that may impact my/our child’s educational experience and well-being.

I/we grant permission to Craftwell Education to discuss, receive, and share school reports, educational and psychological evaluations, transcripts, and teacher comments. Additionally, Craftwell Education may receive personal information from psychologists, therapists, or physicians that could be beneficial in understanding and supporting my/our child's holistic development. All shared information will be used exclusively to guide academic planning and social-emotional support.

I/we understand that recommendations prepared by teachers and other professionals for educational applications must remain confidential. I/we acknowledge that Craftwell Education will not share those documents with me/us.

Once my/our child is enrolled in a new school, I/we authorize that institution to provide Craftwell Education with future grade reports, teacher comments, and test results. Additionally, I/we grant permission for communication regarding social-emotional well-being to ensure continued support and a positive transition.

This authorization will remain in effect for two years unless revoked in writing. A photocopy or electronic copy of this authorization will be considered as valid as the original.
