If you are having a behavior or financial problem with your pet, would you consider keeping him/her if we could help you with training, or direct you to a network of individuals that supply food and other pet items to those who need them? If this is something you are interested in, please ask to speak to a behaviorist (for training/behavior advice) or manager, and we will be happy to provide you with other options before you relinquish your pet. We understand the decision to surrender your pet is difficult. Please note that submitting this form does not gurantee that we will be able to take your dog, and is solely a way for our staff to learn about your animals needs and how we can futher help you and your pet. Surrendering your pet is final and you will not be able to take your pet back. Because we are a non-profit organization, you may be asked to leave a donation to help with their care. We can only accept animals as space becomes available.

I certify that I am the rightful owner/keeper/caretaker/custodian of the animal(s) who is/are the subject of this Animal Surrender Form, hereinafter referred to as “the animal.” I hereby surrender any and all property rights of the animal. I release and discharge ARC from any and all claims, obligations, liabilities, and causes of action whatsoever arising out of or relating to the ownership, possession, or disposition of the animal(s) and agree to hold harmless ARC from and against any and all such rights claims, obligations, liabilities and causes of action which may be asserted by other parties.I certify that no other person has a right of property to the animal. I understand that by surrendering my property rights to the animal, the animal may be transferred into the custody of an animal shelter.I certify that the animal has not bitten or scratched a human or another animal within the past 10 days.I certify and understand that once I relinquish the animal, the animal will not be returned.I certify that I have read and understand the terms of this Animal Surrender Form.
