Emergency Medical Technician Certification Course Registration Form

This form is to register for the Spring 2026 EMT course that begins on February 3rd. Payment, if applicable, for this course is $150 and will be due no later than Saturday, November 1st, 2026 at 5pm. This is to ensure all materials are purchased and received by the first day of class. 

Students must be at least 16 years of age to enroll in the course. Individuals who are 15 years of age may enroll, but will need to be approved by the Virginia Office of Emergency Medical Services and the National Registry of Emergency Medial Technicians. Information on how to receive approval will be sent to you via email if needed. Any student under the age of 18 will receive a parental consent form to be completed prior to the first day of class. 

This course will also be using the Team Reach app for communicating. Please be sure to download it on your mobile device. 

By signing this application, you affirm that all information entered in this application is true and accurate to the best of your knowledge. Any falsification will result in the denial of your registration application and you may face other penalties as well. By signing, you also affirm understanding that your seat in the course is not held until payment is rendered in full. You also understand your enrollment in the course is conditional pending the results of your background check (information on how to obtain background check will be provided on the first day of class).
