The Sweet Blue Foundation is committed to making a lifelong impact for children and their families, and we are eager to learn more about your experience with swim lessons at Sweet Blue Swim Academy.

The survey enclosed below will inform us of the impact that our education programs have on children who have already received a scholarship, and it will help us evaluate swimmers to re-qualify them and, ultimately, determine eligibility for continued support.
