We’re sorry to hear you’re looking to cancel your membership at UNITY Fitness. Please complete the form below to begin your cancellation request.

As outlined in your membership agreement, all cancellations require 14 days’ written notice-submitting this form counts as that written notice. If your next billing date falls within the 14-day notice period, one final membership payment will be processed, and your membership will remain active for that additional month.

A member of our team will follow up with you within 10 business days of receiving your completed form to inform you of the status of your cancellation request or advise if further action is required.
