Please read before agreeing

In consideration of being allowed to participate in any way in the TOA Collective Softball league, related events, and activities, I, the undersigned, acknowledge, appreciate, and agree that:

1. Risk Acknowledgment: Participation includes the risk of serious injury, including permanent paralysis and death. While rules, equipment, and discipline may reduce the risk, serious injury remains a possibility.

2. Assumption of Risk: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the "Releasees" or others, and I take full responsibility for my participation.

3. Compliance with Safety Protocols: I agree to follow all stated and customary safety rules. If I observe any unusual or significant hazard, I will remove myself from participation and report it immediately.

4. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless the TOA Collective, Seattle Parks & Recreation, Guardian Softball League, its officers, officials, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the event (“Releasees”), from any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise.

5. I consent to the use of my photos in any promotion materials of TOA Collective without compensation.

6. I will display good character and athletic behavior inside and outside the playing fields. I agree to pay any property or indemnity damage caused by me, whether voluntarily, negligently or otherwise.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY WITHOUT INDUCEMENT.
