Submission: Please fill in and sunmit this online form. Confirmation emails will be sent by the end of June. Language: Please fill the form in using either Chinese or English.

費用 : 全免（參加者須於報名確認後繳交按金港幣500元正，參加者整體出席率須達到80%，並於測驗合格，方獲於完成計劃後全數退回按金。） 
Fee: Free of charge, however a HKD500 refundable deposit will be collected upon acceptance. The deposit will be returned upon successful completion of the programme, which requires a minimum of 80% attendance and passing the final assessment.

1. 本人(家長) 同意 敝子弟(18歲以下, 下稱 參加者) 參加Young Environment Ambassador Programme (下稱計劃) 所舉辦之室內活動和外出考察 ，並願意自行承擔所有風險及責任。　
   I, as parent or guardian of the participant, hereby give my consent for my child to participate in the Young Environment Ambassador Programme, including all the related workshops and field trips at our own risks and responsibilities.
2. 本人謹此同意免除及解除向承辦機構、各贊助商及其他直接或間接有關人士或機構就參加者因本計劃而引起或導致的損害、損失、申索的責任, 及追究保障及彌償的權利。
   I hereby give my consent, waiver and release and indemnity for or against the Programme Organiser (Civic Exchange), the sponsors and any other individuals or organizations connected directly or indirectly with the Programme in respect of any damage, losses, claims and liabilities arising from or in connection with the Participant’s participation in the Programme.
3. 本人確認本表格及隨附文件所提供的資料均屬正確，並無缺漏。
   I certify that all the information provided above and all the enclosed supporting documents are complete and accurate to the best of my knowledge;
4. 本人允許參加者出現於本計劃有關活動的影片及相片作宣傳之用(如海報、電子通訊及網頁)。
   I allow my child to be photographed/videotaped for the purposes of promoting the programme.
5. 參加者如有特別健康 / 心理狀況，本人會事先通知負責職員。
   If my child has a special medical / psychological situation, I will notify the Programme Organiser in advance.
6. 本人同意於成功遞交報名表後，將參加者以上的個人資料用作承辦機構的聯絡及通訊用途。
   I hereby give my consent for my child’s personal information given above to be released for contact and communication purposes of Civic Exchange, if my child’s application is successfully submitted;
7. 本人同意參加者提供之個人資料將用作參與活動的相關用途、簽發收據、收集意見、資料分析，及其他配合本會宗旨及使命的事項。本人及參加者有權查閱及更改個人資料，如需查閱或更改個人資料，可電郵至 info@civic-exchange.org。
   I agree that my child’s personal data will be used for the purposes related to the participation in different programmes and activities, the issuance of receipts, collecting user feedback, conducting analyses, and any other initiatives in relation to the aims and objectives of Civic Exchange. My child and I have the right to request access to and correction of my child’s personal data. For inquiry or updating personal data, please email to info@civic-exchange.org.
