CDSL Co-Ed Registration

CDSL CO-ED Sign Up

Use this form to sign up for the Cornwall District Soccer League's Co-Ed soccer league, commencing on June 5th 2025.

Co-Ed League Information:
- The cost per player is $100 for 10 weeks.
- Ages 16 and up.
- 11 v 11 full field.
- Max 64 registrations.
- Matches take place every Thursday at the Gray's Creek Field between 6pm - 8pm.
Important - Please Take Note:
- E-transfers are the only form of payment accepted.
- All e-transfers are to be sent to: secretary@cdslsoccer.com.
- When sending the e-transfer, type the name of the player being registered in the 'Message' section. This will help us pair payments with registrations. Take note of the confirmation number after sending.
- On the final page of this registration form, you will be required to provide the e-transfer confirmation number.
For players under the age of 18: a second, separate form will be required to be filled out by your parent and/or legal guardian. This form can be found here. Please fill out the PDF and email it to secretary@cdslsoccer.com with the player's name as the subject.
